People participating in Leadership Academy have several things in common. They are consumer/survivors/ex-patients (CSX) who wish to gain education, new skills and share with others ways to be an effective member of the CSX "voice" in the Mental Health System.

1. Please tell us about your experience with the Mental Health System.

2. Please tell us about any leadership activities in which you have participated (planning club projects, member of boards or committees, etc.)

3. The Leadership Academy is conducted over a three to four day period, with a full morning and afternoon of instruction in a classroom setting. Please tell us how you are prepared for this physical, emotional, and intellectual challenge?

4. Participants in Leadership Academy are ready to move beyond their own personal difficulties. Classes usually have 20 to 25 participants. Please tell us how you are prepared to participate in group discussion, and share in a way that addresses issues as "community needs" not "personal needs".

5. Can you commit to travel, four days away from home, and attending all of the classes?

6. Do you have any special accommodations or dietary needs? Please explain.
